Also, are railings placed properly? should they be offset from edge of stairs and walls?

Too wide.

Too wide. Per code the width of the stair should be 48" clr. (due to refuge Area) + 4" or railing on either side give me a total width of 56", I made it 60". Is it a deal breaker?
