lobby has no exit. Major error at the least. The current west facing door is not in the lobby.
Is there a window between R and CR?
Upper level TR's have no doors. TR's do not have to stack.
Why do SM and LM have 2 doors?

Yup, forgot the lobby door, window between R/CR and doors at upper level TR's.
SM and LM have two doors to provide the 'near' requirement. Wanted to make sure it was less than 30' from door to door per the discussion here:

There is no "30'=near" requirement in the program, and they even say they don't know the grading algorithm. You must go by what is in the program, and the "moderators" from NCARB often make mistakes on Google+.
