4.0 Stair Vignette Attempt

Attached is my stair vignette. Please feel free to leave comment.
1. Is it ok I used stair width of 60" instead of 56" = 4"+48"+4"?
2. Is it ok for the handrail extension at landing 1'9" is overlap with the 12" strike side maneuver clearance in front of the door?
