Ramp Vignette First Attempt

First attempt on ramp vignette. Need some answers.
Is the 12" railing extension required on the top landing from the ramp?
Are the gaps between the 4" high landing and the stair/top ramp allowable?
I did not extend the railing from the ramp to the 4" high landing as I it was an inside handrail on a switchback but I did not continue the handrail down as the landing is less than 6" high.

Yes.

That and the other gap are never a good idea. All spaces should be accessible.

You still need guardrails.

Missing rails.
Missing extension.
