Stair is too narrow.
Janitor CFA is too shallow.

Please confirm if I did the calculation correctly: take the occupancy / number of exits * 0.3 for each space; then use the largest or minimum per code of 44". I used 44" wide and let handrail protrude into the 44" space. Is that wrong?

Yes. Read the entire code.

I think I see it now: "When areas of refuge are required, stairway width shall have a minimum clear width of 48" between handrails."
