First off, thanks for the comments Coach, very helpful. Please take a look at attempt #2 and let me know your comments. Thanks in advance!

Made a few adjustments based on those comments
-stairs are 56" wide (2nd floor + janitor = 189 * 0.3")
-used cut stairs on 1st and 2nd floor views
-stopped handrails at cut lines
-adjusted railings to be 2" from wall
-1 area of reafuge.

I hope that's not how you determined required width.

Coach,

Thanks for the comments. Quick follow up:

-That is how I calculated the required width for the stair. But now I can see that I did that wrong...2nd Floor 180 people /2 exits = 90 people. 90 *.03 = 27" well below the minimum of 44". I had forgotten to divide the occupant load by the number of exits.

-I definitely took your previous comments to heart and make sure the handrails are 2" off the wall surface, though it may not seems like it.

Read the entire code for correct width.
Hold rails 2" off walls AND edges.
