Cricket Ridge Length

Is the use of a 45 degree angle in plan on the cricket related to the roof slope being 6:12 or should we draw all crickets with a 45 degree angle in plan (with the ridge length half that of the chimney base width), regardless of slope?

Yes. The 45 degree angle has nothing to do with 6:12, it indicates that the cricket slope matches the roof plane slope.
