1- does the the rail need to clear the required clearance for the door at the first landing as I have shown here?
I have seen many solutions where it is overlapping
2-is it safe to say that you can draw a 5' wide stair and ramp for both these 4.0 vignettes as acceptable solutions
its much easier and ensures clearances are met and looks clean.

Rail mounted on door wall may encroach.
