Door is too small.
Illegal slope.
Egress narrows at base of stair.
Rail extensions missing at top.
Hold rails 2" off walls and edges.
Stop landing wall rails at extensions.

can i ask for a couple of clarifications?:

Egress narrows at base of stair


i have seen this comment a few places and am still not sure what it means. does it refer to the width of the ramp?

Rail extensions missing at top.


should these turn the 90 degree corner into the exit corridor?

No. It has everything to do with the width of the stair. The path from the stair to the exit can't narrow. The distance from the end of the west rail to the north wall can't be less than the overall stair width.
