Stair Vignette- Specific Clarifications?

Hi guys- Was hoping someone could clarify the required stair width for the 4.0 Stair Practice Vignette.

Doing the occupancy calc: Ground - 36", Intermediate - 2.7", Second - 27"... but all of these are below the code required minimum of 44". But this is even trumped by the 48" clear between handrails for the area of rescue needed on second floor. My question- since the stair width cannot be reduced as you egress, does this mean all stairs need to have 48" clear between handrails (say 56" total width with handrails)?

Secondly- can handrails encroach in door maneuvering space?

Lastly - how does one determine if the door is not reducing the egress path by half? Say there is a 36" door and a required egress of 48"... does that mean there has to be 24" between the edge of the door in the open position and the handrail?
