HOW did you verify the minimum slope?
Grade changed within a tree line. MAJOR error.
Display is 102' below grade. Fail.

Here is the revision! Does this look correct?
I got the minimum slope by setting up a proportion. If the max was 20%. I calaculated 20/100= 1(spacing of the contours)/X. X=5. Then I drew 5' diameter circles. Same for the min 2%. 2/100=1/X. X=50.

There are no sketch elements showing that you checked the minimum slope. Your math is correct, but how did you verify in on the plan?
