Can anyone please help clarify a method to facilitate good documentation? Is it having an office standard? Or planned coordination meetings? Or initial review before drawings are issued? Which is more important?

Regular coordination meetings. That will save you the most headache in the long run. Because coordination takes a bit of time and as long as everybody is working together and on the same page you can avoid costly delays in document production.

Office standard is a no-brainer, everyone should have that.
Initial review before drawings are issued...hopefully you won't be reviewing them for the first time right before they're issued. They should constantly be reviewed and revised (i.e. 50%, 75%, 90%, etc.).
